SKR - Policy Agreement Notification

COVID-19
I/We have read and understand that...
... due to COVID-19, many items in this Agreement Text may be obsolete until further notice. They are kept in tact for future use.
...in response to the COVID-19 pandemic, SKR will be strictly enforcing safety protocols following the Ontario Dance and Performing Arts Studios Workplace Guidelines for COVID-19.
...in response to the COVID-19 pandemic, SKR will be following the mandated guidelines provided by the Ontario Ministry of Health.
...failure of a student or parent to adhere to the set guidelines will result in removal from class and possibly the remainder of the course.
...SKR has devised a series of 2 month sessions to offer financial security to both the SKR company and its clients.
...2 month sessions are therefore not refundable
...SKR has introduced an online portion that can be used for those who can not attend class in person, due to illness or experiencing COVID-19 symptoms.
...payments for each session are due in full before classes start. Families with more than one dancer can opt to make 2 separate payments of 50% each. These are due the 1st of the month.

MEMBERSHIP
I/We have read and understand that a membership fee must be added to each participant.
Membership Fee ($35 per dancer - valid for the year in which it is purchased.
This keeps student tuition costs low as our team works behind the scenes.
The Membership fee is non-refundable and is used in the following way:
- maintaining our studio-class-management system
- maintaining our website
- maintaining studio communication
- offsetting insurance fees
- offsetting music license fees
- etc.

SHOW
I/We have read and understand that...
... dancers who can not attend show rehearsals and performances must opt out of the show in writing before costumes are ordered.
... dancers who do not opt out will be responsible for show fees related to each class they attend.
... costume sizing must be confirmed via Google Forms (emailed to members in the fall).
... all course fees are due upon registration.
... show fees (e.g. costumes, jewelry, and tickets) are non-refundable.
... If costume manufacturing defects occur, SKR will be the sole liaison with the costume company.
... only SKR will troubleshoot any costume manufacturing problems with the supplying companies if needed.
... SKR is not responsible for costume measurement or sizing errors provided by members.
... costume sizing instructions must be read carefully to select the dancer's appropriate costume size.
... all recital dancers must adhere to the dress code.
... all recital dancers (females 7 and older) must be properly equipped with performance make-up, and performance jewelry. (see Recital Package)
...the show fee is a blanket fee that allows for equitable participation in the show - it helps cover costs for many related production costs.
...it also includes 1 costume per course, 1 photo shoot per course and video of the dancer's show performance.
...show fees can not be pro-rated for whatever reason.

FINANCIAL POLICY

... participation in dance classes requires a financial commitment.
... all dancers must be properly equipped with dance shoes, dance wear, and hair accessories in order to participate in class. (see Dress Code)
... my/our registration will be considered pending until all fees are received or instalment schedule is confirmed.
... my/our registration constitutes a contract to pay all fees regardless of dropping classes after the refund request deadline. (see refund policy)
... the Deposit and Membership Fees must be paid upon registering. (minimum payment to hold your spot)
... Discounts/Coupons have expiry dates that will not be extended under any circumstance.

ADDITIONAL ADMINISTRATIVE FEES

... (AF1) An Admin Fee of $15 will be applied to unpaid balances on October 1.
... (AF2) A second Admin Fee of $15 will be applied to unpaid balances on November 1.
... (AF3) A third Admin Fee of $15 will be applied to unpaid balances on December 1.
... (AF4) A fourth Admin Fee of $15 will be applied to unpaid balances on December 15.
... (BO) Balances Owing starting December 28 will be subject to a penalty of 5% of the balance owing (or a $15 Admin Fee - whichever is greater)
... penalties and/or Admin Fees will continue to be applied on the 1st of each month until the balance is paid in full.
... unpaid accounts October 15 will result in revoked access to scheduled classes until accounts owing are paid in full.
... costumes and recital privileges will be withheld until accounts owing are paid in full.
... to ensure that year end payment is made, a one-time credit card transaction can be requested. A 4.25% fee is charged.

CANCELLATION
I/We understand that...
... SKR reserves the right to cancel any class or course for whatever reason (this would be posted on the website or emailed to clients).
... as is common practice in organized activities, no refunds will be given after October 1.
... a refund request must be submitted in writing, via email, on or before October 1.
... A $35 administrative fee per dancer, a $20 per class trial fee, and coupon values will be subtracted from all requested refunds.
... refunds will be sent via eTransfer approximately 1 week after a request is made.
... refunds will NOT be given for individual days of absence for whatever reason. (In case of bad weather, please drive safely or remain at home.)

SUPPORT & PERFORMANCE POLICY
I/We have read and understand that...
... a positive educational experience requires support both in the studio and at home.
... safety and integrity is of utmost importance.
... an annual performance is a special and exciting tradition for dancers that requires 100% participation and support.
... I/We (the dancer and parent/guardian) assume all commitment and support responsibilities for participation in registered activities at SKR.
... regular attendance, punctuality and class etiquette are very important.
... dancers who miss more than 4 classes may be asked to book a private make-up session to ensure safe practice. 
... students will not be allowed on stage if they pose a safety concern to other dancers.
... photos and or videos of dancers may be taken and used for educational and or promotional purposes.
... access to SKR videos does not give anyone the permission to share or circulate these videos.
... SKR has my/our express consent to send me, my child/ward electronic messages.
... studio information can be found on the SKR website and in the studio lobby.
... it is my/our duty to be fully aware of the SKR Timelines, Rules & Regulations, and Policies & Procedures.
... all SKR Studio Dancers MUST attend all Rehearsals and Shows in order to participate in the Recital.
